The form has two layouts. It can be used for anything from surveys, to signup forms, to contact us forms asking for secondary information after a signup.

Layout

When you create the page, the 'Layout' section asks for the 'Form layout'. Pick 'Full page form' or 'Sidebar form'.

Full page

Choosing a full page template will give the form the entire page to create your content. In this case, you will likely want to use a text block in the form to add text, image, and video data to your Page. In the 'Form' tab, 'Add New Form Field' has 'Add Text, Images, Headers, & Links...' in its 'Text & Formatting' group.

Sidebar

You can use the sidebar template to limit your form to the sidebar of the page. A sidebar form page has a 'Page Content' section on its 'Details' tab.
